Title: Mathematical Modeling of Drug Resistance Evolution and The Optimal Treatment Strategy in EGFR mutated Lung Cancer

Abstract

Epidermal growth factor receptor tyrosine kinase inhibitor (EGFR-TKI)
is widely used for treatment of non-small cell lung cancer (NSCLC)
patients with EGFR mutation. However, additional mutations eventually
cause the drug resistance, which can lead to progressive disease.  In
order to overcome this problem, we theoretically investigate the
optimal treatment schedule with available targeted drugs that
minimizes the risk of drug resistance and prolongs the time to
recurrence. We developed a computational model incorporating in vitro
data of cancer cell kinetics, and explore the optimal treatment
schedule that prolongs the time of recurrence. In our parameter range,
we found that using drugs efficient for the resistant cells as the
first-line drug can be beneficial in monotherapy. Moreover, we
suggested the suitable time of drug switching. Finally, the results
also indicated that the combination therapy using two kinds of
EGFR-TKIs has a potential clinical application.
